The treatment for large vessel cerebrovascular disease includes platelet antagonists, folic acid (helps decrease homocysteine), and carotid endarterectomy (if there is high grade carotid stenosis). There are ongoing investigations into intravascular procedures for intracranial atherosclerotic disease (but none are proven as of yet).
